The ad, created by The Creative Partnership for Lions Gate, opened with a female character saying "No" and ended with her saying "I want to go home".
It also featured surgical and operation theatre instruments and a voiceover, which said: "Its shocking...and genuinely terrifying."
The film tells the story of a group of young tourists in the mountains of Brazil who find themselves kidnapped by a group of criminals trading in human organs.
A viewer complained that the ad, which was broadcast at 8pm, was "frightening" and "sickening" and could distress children.
Although the ad was to be shown after the 7.30pm watershed, the Advertising Standards Authority (ASA) said the restriction was inadequate because of the graphic images it contained.
The ASA said: "We concluded that if scheduled before 9pm those images could cause distress to children."
In a separate adjudication, the ad watchdog has cleared a TV spot for Birds Eye Frozen food featuring food critic Giles Coren, despite a complaint.
The commercial backed Birds Eye's claims that the process of freezing food stopped harmful bacteria from developing. The ASA said the spot was "unlikely to mislead".
